Welcome to our online intake and congratulations on taking a step towards starting therapy. Our director of intake is available to answer questions you have at any point. Please share any and all information you’d like for us to know. Your answers will allow us to make sure you get support from a therapist that best fits your needs. Also check out the groups we have available, as they are often an excellent complement to client’s individual, couples, or family therapy.
Aside from the first page, all questions are optional. The full process takes about 10 minutes. For security purposes, your answers will not be saved and must be submitted in one sitting.
You are invited to complete this intake over the phone rather than online, if you prefer. To speak with our director of intake call 773-750-3505. We look forward to meeting you and supporting you in reaching your therapeutic goals.
IntraSpectrum literally means “the spectrum within” – IntraSpectrum counselors serve as guides and companions in the process of exploring with our clients their unique internal spectrum of identities, thoughts, feelings and desires. In our work together, we endeavor to help you become more you.
